There are two PDFs for you to download below. The first is a packet containing tax forms, payroll information, consent forms, and a voluntary disclosure form. The second is our full staff handbook. You are responsible for understanding the entire staff handbook. However, for the purposes of completing the questionnaire in this section, you absolutely MUST read Chapter 4 on staff policies and procedures fully and carefully. At the very end of the staff packet process, you will have to complete a quiz that covers some of the items in Chapter 4, so please make sure you read carefully.
Once you have reviewed both documents, you will complete a Google Form using the information in these documents. It is NOT necessary for you to print out the PDF packets of documents, unless you find it easier to understand the information when reading it in a hard copy instead of on the screen. You will NOT need to submit hard copies of these documents to us. All of the information is collected in the Google Form linked below.
